Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to exploding tire and struck-by injuries: (a) Location - Shop: On or about 7/14/10, the employer failed to ensure that only wheels and tires of the same size were mounted and inflated. Among other methods, one feasible and acceptable means to correct this hazard is to properly train workers to ensure that they only attempt to mount and inflate tires and wheels of the same size, in accordance with the manufacturer's safety instructions.
