Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer does not
furnish employment and a place of employment which are free from
recognized hazards that are
causing or likely to cause death or serious physical harm to employees in
that employees are
exposed to the condition(s) listed below:
The employer did not train employees on how to safely pull the spinning
chain from the cathead
when the chain becomes entangled with the drill line.  This violation
occurred on or about
December 13, 2010, at Bronco Drilling Company, Inc., Rig #62, J. R.
Flowers 2 - #5,
Hemphill County, East Hwy. 33 to CR 28 4 Miles, Canadian Texas. Employees
are exposed to
caught-by hazard.
Among other methods, feasible and acceptable abatement methods to correct
this hazard may
include:
1. Ensure that employees are trained and understand the proper use and
operation of the
spinning chain.
2. Establish and implement a written procedure for spinning the chain
operation.
3. Ensure that employees are qualified to operate machinery such as
cathead when pulling
the spinning chain for drill collars and pipes connections.
Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the
abatement date, the
employer must submit documentation describing the actions it is taking to
establish,
communicate, and implement written procedures to ensure that spinning the
chain operation is
safe.tion
