General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to multiple injuries from falling and crushing hazards when employees are elevated on platforms used with a powered industrial truck: a) Maintenance Area in 480 Mill Road Building: A personnel platform used for raising employee did not have a standard guard railing system that covered all open sides of the platform and the platform did not have guarding to prevent employees from coming in contact with the moving parts in the mast, on or about 11/15/2011. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. ABATEMENT NOTE: The requirements for personnel platforms attached to powered industrial trucks are outlined in ANSI/ITSDF B56.1-2005 paragraph 4.17 'Elevating Personnel'.
