Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while working for powered industrial trucks: a) Warehouse: Employees using a Crown high lift truck (order picker) to remove boxes from storage racks used a body belt that was worn through and the inner cords were exposed, on or about February 13, 2012. ABATEMENT NOTE: One feasible and acceptable abatement method to correct this hazard is to a) replace any body belt, lanyard, or deceleration device that has sustained permanent deformation or is otherwise damaged as required by ITSDF B56.1-2005, Section 4.17, Safety Standard for Low Lift and High Lift Trucks. b) Train employees to inspect and identify permanent deformations to any body belt, lanyard, or deceleration device used with the powered industrial truck. No abatement certification or documentation required.tion
