Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck-by hazards during dismantling of a crane boom.
a) Site - Colonial Blvd/Forum Blvd. in Ft. Myers (FL);
Equipment - Sumitomo Pile Driver Crawler Crane - Serial # LS238 - Model #
LS238RH-
5.  On or about 06/28/10 employees were exposed to being struck-by a
section of crane
boom falling during the dismantling process in that:
1. Sufficient dunnage was not placed under the boom peak and under both
sides
of every joint on the boom prior to removing the boom's lower connecting
pins;
2. The pendant lines were not removed and re-anchored to a lower boom
section
prior to removing the boom's lower connecting pins; and,
3. The lower boom connecting pins were removed when the boom was not
resting
on the dunnage, but removed when the boom was in an elevated horizontal
position above the dunnage.
Among other methods, means of feasible and acceptable abatement include
but may not
be limited to:
1. Utilize the dismantling procedures listed in the Sumitomo Crane
Operational
Manual.
2. Utilize the dismantling procedures listed on page 65 & 66 of the Crane
Handbook from the Construction Safety Association of Ontario, 1975
edition.
3. Train and ensure that employees utilize proper dismantling procedures
when
dismantling crane booms.
