Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing, or likely to cause, death or serious physical harm to employees in that employees were exposed to struck-by hazards: On or about September 1, 2010, and at times prior thereto, at this inspection site, the employer failed to: a.secure pressurized lines to prevent displacement, b.remove abrasive hoses from service when the integrity of the hose was in question. Among other methods, feasible and acceptable abatement methods to correct these hazards would be to: a.Require all pressurized lines be secured to an adequate anchorage point with safety chains/anti-whip connectors. b.Implement a Hose Inspection and Test Plan to insure hose and/or worn or corroded couplings are removed from service. Include inspections of all pressurized hoses prior to use, hydrostatic test performed on all hoses at least every six months and inspection records maintained as referenced in the manufactures (Goodall) document, "Proper Hose Use, Care and Maintenance".
