Initial $5670.00 · Current $3969.00 Reduced
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish to each of his employees a place of employment which was free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of overpressurization of pressure
vessels which are
causing or likely to cause death or serious physical harm to employees:
a) The employer did not inspect pressure relief/safety valves to
determine what the effects of
utility failures and pressure excursions had on such devices that were
attached to the Amine
Flash Tank. This violation occurred on or about 05/28/2011 and at times
prior thereto at the
Holly 6 Amine Trains.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
would be to:
Inspect pressure relief valves to determine the presence of flashing
fluids within the valves and
to determine if scoring/galling of the spindle was occurring that could
cause the valves not to
operate properly.
Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of the
citation, the employer
must submit documentation describing the actions it is taking to ensure
that proper operating
conditions are evaluated and incorporated into the design of all pressure
relief, pressure safety
and similar such devices.

employer does
not furnish to each of his employees a place of employment which was free

harm to employees in
that employees were exposed to the hazards of the rupture of a process
vessel due to over
pressurization:

requirements
of the
Amine Flash Tank to withstand the most severe pressure conditions (1440
psi) that could be
experienced during normal operations. This violation occurred on or about
05/28/11 at the
Holly 6 Amine Trains.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
