Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of: The employer does not ensure that employees apply for the appropriate Safe Work Permits when performing non routine tasks such as connecting a pressurized 1800 psi nitrogen cylinder to sampling line. In an effort to assist in the DeRime process on the Refined Argon Day Tank, on or about 8/5/2011, employees were allowed to connect the 1/8 inch sampling line to the high pressure side of the nitrogen cylinder without a Safe Work Permit. This violation was documented on January 27, 2012. Among other methods, one feasible and acceptable means of abatement of this hazard would be to follow Air Liquide Safe Work Permit Level 2 B Procedure Number 2B-LI-HS-1010-P, Revision: 0, Dated May 31, 2007.
