Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards:
a)Building #3, Graphite Furnace #25 - The six foot stepladder was used in
the
closed position to enter #25 Graphite Furnace, contrary to the
manufacturer's
written directions for safe use. The spreader was not used to hold the
front and
back sections of the stepladder in the open position.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is to comply witht he manufacturer's requirements and ANSI
A14.2.
b)Building #3, Platform, #25 Graphite Furnace - The mobile ladder stand
was not
used as it was designed to be used.  The top step was too high to safely
access
the platform.  Employees climbed over the side of the railing of the
mobile ladder
stand to access the platform.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is to comply with the manufacturer's requirements and ANSI
A14.7.ements
