Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:
The employer does not ensure that employees are protected from the dangers
of struck-by and
caught-in-between-hazards when engaged in rig-up/down operations. On or
about March 12,
2011 an employee was fatally injured when struck and caught-in-between a
forklift and a
tractor's trailer.
Among other methods, feasible and acceptable means of abatement for this
hazard include, but
are not limited to, implementing the measures outlined in the IADC
(International Association
of Drilling Contractors) Safety Meeting Manual, AESC (Association of
Energy Service
Companies) Green Book, installing back-up alarms on tractor trailers and
other
motorized
equipment, establish and implement standard safe work procedures for
employees and/or
operators working near or on moving motorized vehicles.
Pursuant to 29 CFR 1903.19, within ten (10) days of the date of this
citation, the employer must
submit documentation showing that it is in compliance with the standard,
including describing
the steps that it is taking to ensure that employees are protected from
struck-by and caught-in-
between hazards.
