Section Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrical hazards and aerial lift failure. (a) Location - Broadway Street, Haverhill, MA: On or about 9/2/10, the employer failed to maintain periodic inspection on a Reach All vehicle-mounted elevating and rotating aerial device. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS:" 1. The employer shall conduct periodic inspections on all insulated aerial lifts used for tree trimming operations. Vehicles, which have not received periodic dielectric inspections should not be dispatched to work crews working within 10 feet of power lines. (Reference: ANSI A92.2-1990, paragraph 8.2.4, American National Standard, Vehicle-Mounted Elevating and Rotating Devices) "ABATEMENT DOCUMENTATION REQUIRED FOR THIS CITATION"
