North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard: a)jobsite - on July 27, 2010, employees were using a 1" bull rope with a 10-lb, 5", 4 1/2 ton WARN snatch block to drag logs out of a 20' right-of-way. The snatch block was designed to be used with a 3/8" to 1/2" wire or synthetic rope. ABATEMENT NOTE: Among other methods, one feasible abatement method to correct this hazard is to follow the manufacturer's recommendations for the size of rope to be used with a particular sheave/snatch block. Employee training in proper use of snatch blocks would reduce the potential of them using the incorrect size rope. North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard: a)jobsite-on July 27, 2010, employee used a Charleston Rigging Co. 2", 2-ply reversed-eye synthetic sling in a choker hitch to attach a WARN snatch block to an anchor point, two hackberry trees. The sling was twisted and worn. The snatch block struck an employee, killing him instantly. ABATEMENT NOTE: Among other methods, one feasible abatement method to correct this hazard is to -ensure slings are not twisted when used. Twists reduce the weight capacity of the material -conduct periodic inspections with record keeping to ensure slings with defects are removed from service -individually mark slings to maintain record keeping and inspection -provide employee training on proper sling use References: Charleston's Rigging/Carolina's Rigging application and warning for equipment use ASME B30.9, 2006, section 9 sling label-warning and inspectioning

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck hazard: a)jobsite - on July 27, 2010, employees were using a 1" bull rope attached to a 3/4" bull rope. The 1" bull rope was run through a snatch block which was designed for a rope no larger than 1/2" diameter. Both ropes showed significant wear such as volume reduction, up to 25, with pulled cords, frayed threads and small cuts. ABATEMENT NOTE: Among other methods, one feasible abatement method to correct this hazard is to follow manufacturer's instructions for rope inspection and guidelines for removal of rope from service when worn.
