furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fire hazards:
a) Specialty Promotions dba Specialty Print Communications - Employees are
exposed to fire
hazard(s) when combustible paper particulate solids were permitted to pass
through an air
moving device (fan) with no provisions to prevent potential ignited
material from entering the
duct system and trim separator (GF Puhl trim separator) downstream in the
baler room.
General methods of abatement, among others is to follow the guidance
outlined in National Fire
Protection Association (NFPA) 654, Standard for the Prevention of Fire and
Dust Explosions
from the Manufacturing, Processing, and Handling of Combustible
Particulate Solids, 2006,
relating to the minimization of the occurrence and resulting damage from
fire in areas where
combustible particulate solids pass through an air moving device.
Specifically, section 7.12.3,
states that provisions shall be made to prevent ignited material from
entering processes
downstream, in accordance with Chapter 10.
In accordance with 29 CFR 1903.19(d), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
purchase or repair of the equipment, photographic or video evidence of
abatement, or other
written records.
