29 CFR 1926.64(f)(4): The employer did not develop and implement safe work
practices to
provide for the control of hazards during operations such as
lockout/tagout; confined space
entry; opening process equipment or piping; and control over entrance into
a facility by
maintenance, contractor, laboratory, or other support personnel:
(a) On or about July 17, 2010 - Benjamin Moore & Company, 109 Bamberg
Drive, Pell City, AL, prior to the starting of a job that required a hot
work
permit, such as cutting pipes in a process safety management (PSM) covered
process, the employer did not perform a proper evaluation and control of
the
hazards by applying lockout/tagout on equipment that could release chemical
energy in the form of flammable vapors resulting in an explosion.
