Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to falls and burn hazards: (a)On or about 10/18/10 - Furnace Area, the Caterpillar IT14G Front End Loader was not removed from service when it was evident that repairs needed to be made. Among other methods, one feasible and acceptaqble method to correct this hazard would be to remove the equipment from service and make the necessary repairs to the equipment, and to perform daily, the maintenance requirements in accordance with the "Operation and Maintenance Manual, "Maintenance Interval Schedule", Pages 74-75 of the Manual.e",

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to struck-by and/or crushing hazards: (a)On or about 10/18/10 - Pit Area, the safety latch for the hook on the 3-Ton Pit Hoist was damaged and in need of repair. Among other methods, one feasible and acceptable method to correct this hazard is to install a safety latch on the hook in accordance with ANSI B30/10-2005, Section 10-1.2.1.2(c)(4).

Among other methods, one feasible and acceptable method to correct this hazard is to ensure that the rated load of the hoist is marked in accordance with the requirement in ANSI B30.16d, Section 16.1.1.
