Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to a struck-by hazard: a. On or about 10/21/10 - maintenance shop, the one ton Budgit underhung hoist was not marked with the rated load. b. On or about 10/21/10 - welding shop, the Dayton underhung hoist was not marked with the rated load. Among other methods, one feasible and acceptable method to correct this hazard would be to comply with the requirements of the American Society of Mechanical Engineers Standard, ASME B30.11-1998, Monorails and Underhung Cranes.ng

On or about 10/21/10 - welding shop, the hook on the Dayton underhung hoist was missing the safety latch. Among other methods, one feasible and acceptable method to correct this hazard would be to comply with the requirements of the American Society of Mechanical Engineers, ASME B30.11-1998, Monorails and Underhung Cranes, Section 11-2.1.3.3, Adjustment, Repair, Replacements.
