The employer failed to protect an employee, as follows:
a.At the Smith's Center for the Performing Arts Project, 474 South Grand
Central
Parkway, Las Vegas, NV 89101, an employee was standing on wooden planks
installed on a scissor lift to gain extra height while installing
insulation on the
ceiling. By doing so, the employee did not maintain firm footing on the
platform
floor of the scissor lift.

Para 8.10.1 PERSONNEL FOOTING: Personnel shall maintain a firm footing
on the platform floor while working thereon. Use of planks, ladders, or any
other devices on the platform for achieving additional height or reach
shall
be prohibited; Para 8.10.11; SkyJack Safety Manual for Operating and
Maintenance Personnel.
One feasible and acceptable means of abatement method is to ensure that
employees follow all safety rules on the proper operation of scissor
lifts.
