Nevada Revised Statute 618.375(1):  Duties of employers.  The employer
failed to furnish
employment and a place of employment which was free from recognized
hazards that were
causing or likely to cause death or serious physical harm to his employees:
The employer failed to furnish employment and a place of employment which
was free from
recognized hazards, as follows:
a.At the rigid concrete box extension project site located at Milepost 88
on
Highway 6 (between Tonopah and Ely), an employee was exposed to crushing
hazards when he had his leg underneath a load. The employee was using a
tool,
e.g., bar, shovel, pick, to knock frozen dirt off the bottom of a concrete
box that
weighed 20,450 pounds. The box was elevated approximately four feet above
ground by two front end loaders with fork attachments.

Precautions
Page 1-3-4: Be careful when lifting objects. Never attempt to lift objects
too
heavy for your machine. Assure machine stability and hydraulic capability
with
a test lift before attempting other maneuvers. Use an adequate chain or
sling
and
proper rigging techniques to attach and stabilize loads. Never lift an
object above
or near another person.
One feasible means of abatement is to make sure that no workers are near
hazardous objects while the objects are elevated.
