recognized hazards that are causing or are likely to cause death or serious
physical harm to his or her employees, as follows:
1.An employee operates press brakes and the employer did not establish a
program for the training of operators and supervisors. The employer also
did not establish a program for inspection of the equipment.
2.Employees operate shears and the employer did not establish a program
for the training of operators and supervisors. The employer also did not
establish a program for the inspection of the equipment.
3.The foot pedal for the Chicago Model 410-D press brake was not
protected from unintended actuation.
4.The foot pedal for the Niagara IL-12 shear was not protected from
unintended actuation.

of power operated
back gages; 3) Control configuration for the shear; 4) Location and proper
operation of energy
isolation devices, including electrical, hydraulic, pneumatic, etc.; 5)

reporting to the proper
supervisor any condition concerning the shear or workpiece operation that
may affect the safety
of individuals. 3) Maintenance personnel shall be trained in safe work
procedures for inspecting
and maintaining the shear. 4) Supervisors shall be trained in safe work
procedures for set-up,

follow a safe procedure before allowing a shear to be inspected or
maintained. Following the
inspection or maintenance, the machine shall be restored to the
predetermined
safe operating
