The employer did not furnish employment and a place of employment which

recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that the employees were exposed to a crushing hazard from the
Harris HRB 8
baler:
a) Baling area - The Harris HRB 8 baler did not have: a working start-up
alarm; a red
emergency stop button on the control panel (the emergency stop button had
faded to
white); a warning sign located on the access cover warning employees about
removing
the access cover; a danger sign located on the inside and outside of the
loading hopper
warning employees not to enter the hopper; a warning sign located at or
near the
entrances warning employees about following lockout procedures before
entering
confined spaces; and, a danger sign located at or near the entrances
warning employees
about confined spaces.
Among other methods, one feasible and acceptable method to correct this
hazard is to:
1) Install a start-up alarm; 2) Install a red emergency stop button; 3)
Post warning and
danger signs in accordance with American National Standard, ANSI Z245.5
