that were causing or likely to cause death or serious physical harm:
On or about November 29, 2010; A Crown Lift is used with a personnel
basket to raise
employees to a height of 9 to 10 feet to clean filters and lights in the
facility.  The lift and
basket are also used to turn off main power supply to saws and sanders.
The forks of the lift
slide into channels on the basket.  The basket is not secured to the lift
in any other way.
Employees are exposed to the hazard of falls.
Among other methods, one feasible and acceptable method of abatement would
be to secure
the personnel basket to the lift with a chain.
