29 CFR 1910.119(h)(3)(iv): The contract employer did not assure that each contract employee follows the safety rules of the facility including the safe work practices required by 29 CFR 1910.119(f)(4): On or about December 10, 2010, the contract employer did not ensure each contract employee followed the Holly Refinery safe work practice for entering and exiting process areas. Four Brock Specialty Services employees signed in and entered the Unifiner process area but failed to sign out upon exit, exposing employees to the potential hazard of fire, explosion and inhalation of toxic vapor.
