Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm: Well site; on or about April 12, 2011, employees operated a service rig during the reworking of an existing oil well which required the derrick hand to work from the mast mounted platforms; no secondary emergency means of egress was provided on the mast working levels. This exposed the derrick hand to the hazards of fire and/or rig collapse. The American Petroleum Institute Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, RP 54, 6.10.1 requires that an auxiliary means of escape be installed on the rig mast/derrick providing a ready and convenient means of escape from the derrickman's working platform. among other methods, one feasible and acceptable abatement method to limit the employee's exposure to the hazards is to install a emergency escape system in accordance with the manufacturer's instructions and industry practices.ns
