Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm:
The employer violated this standard on or about September 1, 2011 when the
R & S Well
Services, Inc, located that the Clarence Tullius and Frank Tullius Lease
Section 16-8N-2W in
Cleveland County, Oklahoma, did not ensure that employees performing work
in the rod
basket pulling rod were provided with emergency escape device to exit the
mast and derrick
in an emergency.
A feasible and acceptable method of abatement is to provide an emergency
escape device to
exit the mast and derrick in accordance with The American Petroleum
Institute (API),
Recommended Practice #54, for occupational safety for oil and gas well
drilling and
servicing operations, Section 6, Paragraph 10, Auxiliary Escape. This
section
states in
6.10.1: On all land rigs, the derrick or mast shall have an auxiliary
means of escape installed
prior to personnel working in the derrick.
