were causing or likely to cause death or serious physical harm:
Drilling rig drilling floor:  On or about and times prior to September 14,
2011, employees
operating a drilling rig during the drilling of an oil/gas well used the
drilling floor mounted air
hoist to lift drilling pipe and other drilling equipment to the drilling
floor.  The air hoist wire
rope line was damaged in several places with one area severely smashed.
This exposed
employees to the hazard of being struck by a falling load.  Among other
methods, one feasible
and acceptable abatement method to correct this hazard would be to
institute an inspection
program based either on American National Standard Institute (ANSI)
Standard B30.7, Base
mounted Drum Hoist or the American Petroleum Institute (API) Recommended
Practice 54,
Recommended Practice for Occupational Safety for Oil and Gas Well Drilling
and Servicing
Operations, inspection requirements, and removal of the wire rope from
service when it meets
the wire rope manufacture's, API, and/or ANSI removal criteria.re's,
