General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards such as but not limited to fires and explosions while working daily to heat treat parts. On or before September 21, 2010, while performing general maintenance, employees were exposed to explosion and fire hazards on the Lanley Oven (DAC 1), serial number 57240, in that, the Eclipse Manual Control Valve was leaking gas into the combustion chamber while turned off due to improperly seated gas valves. One feasible and acceptable abatement method to correct this hazard would be to follow the National Fire Protection Association (NFPA) standard, NFPA 86-2011, Standards for Ovens and Furnaces, Chapter 7, Safety Equipment and Application by training maintenance personnel to include but not limited to inspections and testing of relief devices and safety interlocks and valves.
