Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to a struck-by hazard: a. On or about 01/26/11 - Fabrication shop, the 5 Ton Load Lifter underhung hoist control box buttons were not marked to identify the direction of loads being lifted. Among other methods, one feasible and acceptable method to correct this hazard would be to comply with the requirements of the American Society of Mechanical Engineers Standard, ASME B30.16-2003, Section 16-1.1.2. The underhung hoist needs to be inspected on a regular basis to ensure that the equipment and controls are in proper working condition.
