in the area
of the mud pump were exposed to struck by hazards created by the whipping
action of
high pressure hoses in the event of a hose failure. The connections of the
mud hoses
were where not protected by safety chains or cables.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS: TO SECURE THE HOSE ENDS USING CABLES OR
CHAINS PROPERLY LOCATED AND OF SUFFICIENT STRENGTH TO RESIST HOSE
WHIPPING SHOULD IT BECOME DISCONNECTED OR BROKEN AS REFERENCED IN
PARAGRAPH 9.13.3 OF API RP 54, RECOMMENDED PRACTICE FOR OCCUPATIONAL
SAFETY FOR OIL AND GAS WELL DRILLING AND SERVICING OPERATIONS, 1999
EDITION.
29 CFR 1903.19(c)(1) requires certification that the abatement of the
above violation is
completed.

the drilling tongs was not installed with enough wire rope clips for the
size of
wire rope used. Two wire rope clips were used on 5/8 inch wire rope.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS: Install a minimum of three wire rope clips when
the size of the
wire rope is greater than 7/16 inch as recommended in Chapter 4 of the
Wire Rope Users
Manual from the Wire Rope Technical Board.
