Section 5(a)1 of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to hazards associated with pressurization of a tank:
Employees performing CIP were exposed to injury in that the Balance Tank
for the Multi-tube
Heat Exchanger became pressurized when the sealed tank was operated at a
temperature above
the design parameter for the tank.  The tank was not vented and was being
operated with the lid
clamped on to the tank and a gasket in place.  On 6/28/11 an employee
suffered burns from
being sprayed with steam and hot water when undoing the clamps on the tank
lid to add
chemicals to the tank during the CIP procedure.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS: Ensure the tank is operated at temperatures within
the
manufacturer design parameters and is properly vented.
29 CFR 1903.19(d)(1) requires certification and documentation that the
abatement of the
above violation is completed.
