Section 5(a)1 of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to fire and explosion hazards:
Employees working on rig #2 at SEC 20, TWP 22, R 33, 990 fnl, 330 fwl,
Tennis,
Kansas were exposed to fire and explosion hazards for the following
instances:
a)Employees were exposed to fire and explosion hazards in that the
employees working on the derrick platform did not have a means of
auxiliary escape.  On the day of the inspection, the auxiliary escape line
was erected, but no carriage/trolley was installed.
29 CFR 1903.19(d)(1) requires certification and documentation that the
abatement of the
above violation is completed.
Among other methods, one feasible and acceptable method to correct this
hazard
is to
install the auxiliary escape line and the escape trolley immediately after
the rig is erected
and prior to personnel working in the derrick platform as referenced in
paragraph 6.10.1
of API RP 54, Recommended Practice for Occupational Safety for Oil and Gas
Well
Drilling and Servicing Operation, Third Edition
b)Employees were exposed to fire and explosion hazards in that employees
working on the drill floor only had one set of stairs to evacuate the
platform in the event of an emergency.
Among other methods, one feasible and acceptable method to correct this
hazard is to
install a minimum of two (2) stairways shall be installed on drilling rigs
to provide
alternate exits from the rig floor to ground level as referenced in
paragraph 9.3.10 of
API RR 54, Recommended Practice for Occupational Safety for Oil and Gas
Well
Drilling and Servicing Operation, Third Edition
29 CFR 1903.19(d)(1) requires certification and documentation that the
abatement of the
above violation is completed.

causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to struck-by hazards:
Employees working near the high pressure mud pump at rig #2 at SEC 20, TWP
22, R
33, 990 fnl, 330 fwl, Tennis, Kansas were exposed to struck-by hazards for
the following
situations:
a)The whipping action of unsecured high pressure mud hoses.  The high
pressure mud hose connection between the fixed pipe on the rig and the
hose from the high pressure mud pump was not protected with chains or
cables.
Among other methods, one feasible and acceptable method to correct this
hazard
is to secure hose ends using cables or chains properly located and of
sufficient
strength to resist hose whipping should it become disconnected or broken
and/or
replace damaged equipment as referenced in paragraph 9.13.4 of API Rp 54,
Recommended Practice for Occupational Safety for Oil and Gas Well Drilling
and
Servicing Operations, Third Edition, 1999, Reaffirmed, March 2007.
29 CFR 1903.19(d)(1) requires certification and documentation that the
abatement of the
above violation is completed.
b)Employees working on the drill floor were exposed to struck-by hazards
when lifting sections of drill pipe.  The hoist line connection to the
chain
only had two crosby clips that had been wedged to together from the hoist
line slipping.
Among other methods, one feasible and acceptable method to correct this
hazard
is to install a minimum of three wire rope clip/crosby clips correctly and
securely
when the wire rope is greater than 7/16 of an inch as recommended in
Chapter
4 of the Wire Rope Users manual from the Wire Rope Technical Board, fourth
edition, December 2005.
29 CFR 1903.19(d)(1) requires certification and documentation that the
abatement of the
above violation is completed.
c)Employees working on the drill floor were exposed to struck-by hazards
when using the pipe tongs.  The tongs were supported by wire ropes using
only two crosby clips.
Among other methods, one feasible and acceptable method to correct this
hazard
is to install a minimum of three wire rope clip/crosby clips correctly and
securely
when the wire rope is greater than 7/16 of an inch as recommended in
Chapter4 of the Wire Rope Users manual from the Wire Rope Technical Board,
fourth
edition, December 2005.
29 CFR 1903.19(c)(1) requires certification that the abatement of the
above violation is
completed.
