Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by an exploding tire and rim assembly: a)1890 Montgomery Highway, Dothan, AL 36303; On or about November 2, 2010 employees were exposed to being struck by an exploding tire and rim assembly while attempting to seat the tire bead. The employer failed to follow the tire changer machine manufacturers Safety and Operating Instructions in that employees were allowed to exceed the maximum 40 PSI air pressure while attempting to seat a bead on a tire. The tire exploded seriously injuring one employee. A feasible and acceptable abatement method to correct this hazard is to follow the safety instructions in the Coats 5065EX Rim Clamp Tire Changer Operators Manual (8182883 04).2883
