Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm or death to employees in that employees were exposed to the hazards of contact with high pressure water: a)At the International Paper Mill located in Pine Hill, AL: On January 6, 2011 and at times prior to, the employer failed to provide training to employees in the safe operation of the hydro blasting pressure spraying machines, exposing employees to being struck-by ultra high pressure water while using the lance to clean industrial equipment. b)At the International Paper Mill located in Pine Hill, AL: On January 6, 2011 and at times prior to, the employer failed to provide two operators for the hydro blasting pressure spraying machines in that at the time of the accident there was no operator at the pressure by-pass switch, exposing employees to being struck-by ultra high pressure water while using the lance to clean industrial equipment. A feasible means of abatement would be to follow the operating instructions for the Jet Stream, Model 4220 Hydro Blaster, which are provided by the manufacturer in the training video and operator's manual (Jet Stream Pump Operation & Maintenance Manual) that requires training of all operators and includes the safety warning to utilize two operators in line of site that have with the ability to effectively communicate.
