Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Bandera Drilling Inc., Rig 1, Well WODC-765, Yoakum County, TX. For the period of time up to and including 04/19/11, employees were exposed to overhead struck-by hazards due to the bales not being removed prior to the commencement of jarring operations. Feasible and Acceptable Abatement Methods would include, but is not limited to: Ensure the bales are removed prior to any operation beginning that does not require the use of the bales.
