Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to rotating spindle: a)New building machine area - On January 13, 2011 and at times prior to employees were exposed to caught in hazard from the rotating spindle on the Bridgeport 1020X, door interlock function was not in operation. Among others one possible and feasible abatement method is to comply with the machine manufacture Operators Safety Manual Section 3 which requires that "Before starting the machine operation, make sure that the door interlock function is valid". b)New building machine area - On January 13, 2011 and at times prior to employees were exposed to caught in hazard from the rotating spindle on the Bridgeport 1000XP door interlock function was not in operation. Among others one possible and feasible abatement method is to comply with the machine manufacture Operators Safety Manual Section 3 which requires that "Before starting the machine operation, make sure that the door interlock function is valid". c)New building machine area - On January 13, 2011 and at times prior to employees were exposed to caught in hazard from the rotating spindle on the Bridgeport Hardinge SR 200M door interlock function was not in operation. Among others one possible and feasible abatement method is to comply with the machine manufacture Operators Safety Manual Section 3 which requires that "Before starting the machine operation, make sure that the door interlock function is valid".n
