LM-EBT8 forklift
attachment with broken safety latches on both hooks to unload 4,000 lbs
bundles of steel rods:
a)4600 N. Park Drive, Opelika, AL 36801; On or about January 25, 2011 and
at
times prior thereto employees were exposed to crush hazards while using a
Vestil
Lift Master Boom LM-EBT8 forklift attachment with broken / missing safety
latches on both hooks to unload 4,000 lbs bundles of steel rods.
A feasible and acceptable method to correct this hazard is to follow the
American Society
of Mechanical Engineers for Hooks (ASME B30.10 - 1993) Section 10-1.2.3
Maintenance, Item c., which states; "A hook latch when required, that is
inoperative
shall be repaired or replaced."
