employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards associated with:

Workers were not adequately protected against struck-by and caught between
injuries while
walking behind the Gradall excavator.
A feasible and useful method of abatement, among other methods, is the use
of barricades or
flagged warning lines to prevent unauthorized worker access into the
danger zone.r
