A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious harm to their employees, in that employees entered confined spaces without testing the internal atmosphere prior to entry to ensure safety. a)22223 N. 16th Street, Phoenix, AZ, 85024: Employees entered confined spaces (sanitary sewers) where the internal atmosphere had not been tested, with a calibrated direct-reading instrument, and found safe for oxygen deficiency, presence of explosive gases and vapors and for potential toxic contaminants.
