Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: serious chemical irritation of the pharynx due to ingestion of sodium hypochlorite 10% by weight solution: a)Location: Willow Springs Housing Project, Kaibeto, AZ 86053: There is no working siphon pump and employees were exposed to corrosive chemical contact when siphoning sodium hypochlorite 10% solution by mouth through a garden hose to transfer the chemical from a 55 gallon drum to a 5 gallon plastic bucket. The sodium hypochlorite was then poured from the bucket into a 70,000 gallon water storage tank, where employees were required to ascend a fixed ladder and stand on the top unguarded surface of the storage tank, 31 feet above the next level, and manually pour the solution into the hatch opening atop the water storage tank. Hazard identified on or about 01/04/2011. ABATEMENT DOCUMENTATION AND CERTIFICATION REQUIRED NOTE: ONE FEASIBLE AND USEFUL METHOD OF ABATEMENT WOULD BE TO INSTALL AN AUTOMATIC DOSE INJECTOR PUMP TO TRANSFER THE SODIUM HYPOCHLORITE 10% SOLUTION FROM THE PRIMARY STORAGE CONTAINER TO THE WATER STORAGE TANK.
