Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls from elevations: a)On January 27, 2011, at the site located at 4725 N. Western in Chicago, Illinois 60625, an employee of Olympic Signs, Inc. was using an Elliott Model ECL-3-55 SN: 3108 Hi Reach Aerial Lift (Truck #54) and exposed to serious fall and tipping hazards as he elevated and extended the boom of the aerial lift platform without fully extending all of the outriggers of the vehicle. Among other methods, one feasible and acceptable method to correct this hazard would be to comply with the American National Standard Institute (ANSI) for Safety Requirements for Vehicle-Mounted Elevating and Rotating Aerial Devices, ANSI A92.2 Section 10.8 (2001), the manufacturer's operating instructions of Elliot Equipment Company and to ensure that outriggers are fully extended. **In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).**
