Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to fall hazards:
(a)Outside of Building:  Employees were using a portable metal extension
ladder
to access/egress a roof where exposed to fall hazards from approximately 13
feet as determined on or about 03/28/2011.
ABATEMENT NOTE: Among other methods, feasible and acceptable abatement
methods to
correct this hazard are:
1. To follow ANSI A14.2-2000 standard, American National Standard for
Ladders Portable
Metal Safety Requirements; Section 8: Selection, Care, and Use, paragraph
8.3.10 Getting
On or Off Ladder at Upper Levels, which includes securing the ladder at
the
upper landing
level and extending the ladder 3 feet at the upper landing level.

that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard to a fall hazard from a work platform
suspended on
the forks of a forklift truck:
(a)Inside the Building: On March 28, 2011, it was determined that an
employee
conducted work from a work platform attached to the forks of a Caterpillar,
model T50B, forklift at an approximate height of 10 feet from the ground
level.
ABATEMENT NOTE: Among other methods, feasible and acceptable abatement
methods to
correct this hazard are:
1.To use a platform that meets the design requirements contained in
American
National Standards Institute Standard ANSI B56.1-2005 entitled "Safety
Standard for Low Lift and High Lift Trucks". In particular, comply with the
following provisions of the standard:
a.Have a qualified engineer evaluate the structural integrity of the
platform to ensure that the platform has a structural safety factor of not
less than 3 to 1 (ANSI B56.1-2005, paragraph 7.37.1(g)).
b.Provide a means to securely attach the platform to the lifting carriage
or forks of the fork truck (ANSI B56.1-2005, paragraph 7.37.3(c))
c.Ensure that the maximum including personnel and equipment, weight of
the empty platform, and minimum capacity of the truck on which the
platform can be used is prominently displayed on the platform.
Note: The combined weight of the platform load and personnel cannot exceed
one-half the
capacity of the fork truck on which the platform is used. (ANSI
B56.1-2005, paragraph
7.37).
