1910.119(j)(4)(iv):  The documentation of the inspection or test that had
been performed on
process equipment to maintain its mechanical integrity did not identify
the serial number or
other identifier of the equipment on which the inspection or test was
performed and the
results of the inspection or test:
(a)Employees were exposed to the hazard of a potential leak of large
quantities of
anhydrous ammonia in that the employer did not have required inspection
documentation associated with their process safety management program.
Documentation of ammonia piping inspections that were conducted in
September of 2007, October of 2007, and May of 2011 contained only the data
points measured and did not include any documented analysis of the piping
condition such as corrosion rates, life expectancy, or any other
information
that rated the fitness for service of the piping sections as determined on
or
about January 10, 2012.
(b)Employees were exposed to the hazard of a potential leak of large
quantities of
anhydrous ammonia in that the employer did not have required inspection
documentation associated with their process safety management program.
Documentation of the 2010 and 2011 annual ammonia equipment inspection
done per IIAR-110 (International Institute for Ammonia Refrigeration -
Startup, Inspection and Maintenance of Ammonia Mechanical Refrigeration
Systems) did not identify the vessels inspected using the identifier
numbers
used by the Marmet, WV site as determined on or about January 10, 2012.
(c)Employees were exposed to the hazard of a potential leak of large
quantities of
anhydrous ammonia in that the employer did not have required inspection
documentation associated with their process safety management program.
Documentation of the 2011 annual ammonia equipment inspection done per
IIAR-110 (International Institute for Ammonia Refrigeration - Startup,
Inspection and Maintenance of Ammonia Mechanical Refrigeration Systems)
did not include the deficiencies found by the inspector as determined on or
about January 10, 2012.
(d)Employees were exposed to the hazard of a potential leak of large
quantities of
anhydrous ammonia in that the employer did not have required inspection
documentation associated with their process safety management program.
Documentation of the 2010 and 2011 annual ammonia equipment inspection
done per IIAR-110 (International Institute for Ammonia Refrigeration -
Startup, Inspection and Maintenance of Ammonia Mechanical Refrigeration
Systems) did not include information on whether several pieces of
equipment passed or failed specific written criteria that was being
evaluated by
the inspector as determined on or about January 10, 2012.

29 CFR 1910.119(o)(3): A report was not developed of the findings of the
compliance
audit
required by 29 CFR 1910.119(o)(1):
(a)Employees were exposed to the hazard of a potential leak of large
quantities of
anhydrous ammonia in that the employer did not maintain documentation
showing that they have evaluated each section of the process safety
management standard to verify that the procedures and practices developed
under the standard are adequate and are being followed.  The employer did
not
develop a report of findings on all elements of the standard in the last
two
compliance audits as determined on or about January 10, 2012.
(b)Employees were exposed to the hazard of a potential leak of large
quantities of
anhydrous ammonia in that the employer did not maintain documentation
showing that they have evaluated each section of the process safety
management standard to verify that the procedures and practices developed
under the standard are adequate and are being followed.  The audit team
assignments for the May 2011 audit did not include evaluating the
requirements of section (o), Compliance Audits, as determined on January
10,
2012.
