furnish employment and a place of
employment which were free from recognized hazards that were causing or
likely to cause death or serious
physical harm to employees:

Condition:An employee was exposed to the hazard of falling while elevated
as great as 9 feet
above ground from a work platform that was not secured to the forklift.

Condition:An employee was exposed to the hazard of amputation while
working within inches of
the unguarded chain hoist mechanism.
Among other methods, one feasible and acceptable method to correct this

provisions of the American National Standard Institutes "Safety Standard
for Low Lift and High Lift Trucks",
ANSI/ITSDF B56.1-2009; section 7.37 Platforms: Elevating.
Abatement documentation (photographs, receipts, copies of programs,
training certification, etc.)
is required for this item.
