Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a) Location: 1000 Western Avenue, Lynn, MA:
On or about 02/11/2011, employees were exposed to explosion hazards when
the
employer failed to establish a program to provide assesment and
documentation of the
condition of the covered piping system.
Among other methods, a feasible means to eliminate or reduce this hazard
is to establish
a program to assess and document the covered piping system as required per
ASME 31.1
and or follow the recognized and generally accepted good engineering
practices
recomended by a third party consultant that conducted a root cause
investigation of the
incident.tion
