On or about April 26, 2011 and times prior thereto, employees of Centria
were working
on the foam line with liquid chemicals such as but not limited to tertiary
amine, N,N,-
dimethylcyclohexylamine, and various diisocyonates.  Employees were
instructed to use
a plumbed eyewash station located in the hall next to the foam line in the
event of an
emergency.  The eye wash station had not been activated weekly to flush
the line,
exposing employee's eyes to foreign material such as but not limited to
contaminated
water and bacteria in the event of an emergency.ed
