General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:
PULLING UNIT-RIG 1342: On or about 3-12-11, the employer did not ensure
that the rig's
equipment was operational and safe to use.
a) The swivel joint that failed when used for well servicing was not
inspected and tested before
use.
b) The kelly hose attached to the swivel joint was not secured to prevent
movement should the
swivel joint fail.
Among other methods, one feasible and acceptable abatement method(s) to
correct this hazard
include, but are not limited to, implementing the manufacturer's
recommendations and to follow
American Petroleum Institute (API) RP-54 practices such as in Section
6.1.1.
Pursuant to 29 CFR 1903.19, within ten (10) days of the date of this
citation, the employer must
submit documentation showing that it is in compliance with the standard,
including describing
the steps that it is taking to ensure that employees are protected from
struck-by hazards when
engaged in well servicing operations.
