Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized
hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the condition(s) listed below:
On or about 08/18/2011 and at times prior thereto, at the jobsite located
at Bassinger Long 5
B Garza County, TX., employees were exposed to crush-by hazards while
working on a pump
jack that had not been secured to prevent unintended movement.
Among other methods, recognized and feasible means to correct this hazard
include, but are not
limited to: The employer can follow the recommendations of the American
Petroleum Institute
Recommended Practice 74 para. 6.3.2.2 which states "During well servicing
operations, the
pumping unit shall be secured to prevent unintended movement. Use of the
brake only or the
brake and brake pawl are not acceptable means of securing the pumping
unit."g
