hazards that were causing or likely to cause death or serious physical
harm to his employees in
that employees were exposed to struck-by or crushed-by hazards:
(a)On or about April 7, 2011 - at the Packaging Area, an employee was
operating
a forklift without using the seat belt.
Among other methods, one feasible and acceptable method to correct this
hazard is would
be to ensure that all forklift drivers utilize the restraint devices
provided on the forklift
as stated in ASME B56.1-1993 - Safety Standard for Low Lift and High Lift
Trucks,
section 7.39, Operator Restraint Systems.t

that employees were exposed to a struck-by and crushed by hazard:
(a)On or about April 7, 2011 - in the Large Diameter Core room, the jib
crane
(underhung hoist) was not marked with the rated load.
Among other methods, one feasible and acceptable method to correct this
hazard would
be to comply with the requirements of the American Society of Mechanical
Engineers
standard, ASME B30.16-2007, Section 16-1.1.1, Rated Load.
