employees, in that: employees were exposed to struck-by hazards due to
damaged below-the-
hook lifting devices and to a lifting device not marked.
(a)On or about December 22, 2011 in the rack-up area at the beginning of
the dip
tank line, below-the-hook bent and damaged lifting devices were not
removed
from
service for repair or replacement.
(b)On or about December 22, 2011 between the dip tanks and the kettle
containing
melted zinc, the Dross Scope was not marked with an ID listing but not
limited to the
following:
- Manufacturer's name and address
- Serial number of the device
- Lifter weight if over 100 lbs (45 kg)
- Cold current (amps when applicable)
- Rated voltage (if applicable)
- Rated load [as described in par. 20-1.2.1(a)]
- ASME BTH-1 Design Category
- ASME BTH-1 Service Class
Among others, one feasible and useful method to correct the hazards is to
follow the
requirements found in American Society of Mechanical Engineers (ASME)
stand ASME B30.20-
2006 for inspection, removal from service, repair, and/or replacement and
for proper inspection
and marking of the below the hook lifting devices.
