Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
On or about March 9, 2011, in the production area at line #3, the
emergency stop actuator
located on the left side of the automated saw down cut (TSD #2) was
missing.  The emergency
stop control was not functioning and readily accessible to disengage the
saw in the event the
operator(s) were to be come entangled, entrapped or struck by broken parts
of the rotating saw
blade.
Among other means, one feasible and acceptable abatement method to correct
this hazard is to
follow the guidelines in National Fire Protection association (NFPA) 79,
2007 Edition, Electrical
Standard for Industrial Machinery, Chapter 10.7 Devices for Stop and
Emergency
Stop as
applicable.
