Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an amputation hazard due to complete maintenance checks not being performed on press brakes. a)For press brakes such as but not limited to the Chicago, Cincinnati, Velson, Promecam, etc; as obseved on or about 06/08/11. Electrical checks on parts such as but not limited to solenoids, relays, starters, controls, motors, etc; were not performed on a daily, weekly, monthly and annual basis. One feasible and acceptable abatement method, among others, to correct this hazard is to follow the manufacturer's maintenance checklists and the inspection criteria listed in ANSI (American National Standards Institute) B11.3-1982, American National Standard for Machine Tools - Power Press Brakes - Safety Requirements for Construction, Care and Use.
